A critical safety breach on SpaceX’s Falcon-9 rocket—meant to carry four astronauts including Group Captain Shubhanshu Shukla—was narrowly avoided thanks to the Indian Space Research Organisation’s (ISRO) relentless insistence on transparency. The intervention not only prevented a possible catastrophe but also highlighted India’s growing role as a global watchdog in space exploration.

Falcon-9 was preparing for launch when ISRO experts noticed irregular oxygen sensor readings. While SpaceX initially dismissed the anomaly as a minor issue, ISRO demanded full disclosure. Out of 14 critical technical queries raised, only two were answered—prompting greater concern.
Hours before the scheduled liftoff on November 11, ISRO’s persistence forced SpaceX to scrub the mission. The subsequent investigation revealed a dangerous crack in the fuel line—a fault that could have caused the rocket to fail mid-flight.

“Had this gone unnoticed, it would have been nothing short of a disaster,” ISRO Chairman V. Narayanan stressed.
